Office Visits / What to Expect
The initial visit at Atlanta Tinnitus Consultants, LLC is devoted to evaluation, explanation, and recommendation. Dr. Nagler will take a detailed history. He will perform a tinnitus-related head and neck examination, including carefully checking for somatosounds ("objective tinnitus"). He will review any records or tests patients bring with them to their appointment. Next he will explain all of his findings in detail and answer any questions. He will spend considerable time differentiating tinnitus fact from tinnitus theory and from tinnitus myth so each patient will be in a better position to make informed decisions in his or her own best interests. The impact of hyperacusis and other forms of sound sensitivity, if present, will be discussed. Finally, Dr. Nagler will offer specific recommendations based upon the patient's evaluation, and he will discuss the rationale behind those recommendations.
The office visit described above typically takes three hours to complete; the cost is $540. Because of the volume of information exchanged, patients often find it helpful to bring along a spouse, partner, relative, or friend.
For some patients Dr. Nagler may suggest additional audiological testing, but only if that testing is felt to be absolutely necessary. In such instances patients are free to obtain the tests anywhere they wish. If desired, Dr. Nagler can arrange to have the testing done on the same day as the initial visit. Atlanta Tinnitus Consultants, LLC has no financial arrangements with any audiologists or testing facilities. If testing is indicated, Dr. Nagler will thoroughly explain all of the results.
In many cases no additional appointments will be necessary. For those who choose to undergo a habituation-based protocol such as Tinnitus Retraining Therapy (TRT), three separate counseling sessions spread out over twelve months are usually indicated. The two-hour primary TRT counseling session ($360) can often be conducted on the day after the initial office visit, which is especially helpful for patients living at a distance from Atlanta. The two follow-up TRT counseling sessions ($120-$240) can be conducted by phone if necessary so that patients living at a distance do not have to travel more than once.
Most TRT patients will require wearable devices during the course of treatment, after which they are no longer needed. These devices are smaller than hearing aids and cannot be detected unless you are purposely looking for them. Atlanta Tinnitus Consultants, LLC does not sell the devices and has no financial interest in their sale, but Dr. Nagler can point you to a few sources where the devices can be obtained on the day of your visit at a very reasonable price.
Clinicians often order an extensive battery of testing in an effort to discover the cause of one's tinnitus or to rule-out other problems. Please bring copies of all tinnitus related medical records and test results (audiograms, scans, etc.) to Atlanta Tinnitus Consultants, LLC at the time of your initial visit. Your records and studies will be carefully reviewed and their significance, if any, explained in detail. We ask, however, that you not undergo any additional testing specifically in preparation for your visit. In general we have found that tinnitus patients are frequently "over tested" or that the testing is done in a manner that does not contribute to a clinical decision specifically as related to tinnitus.
Billing and Insurance
Atlanta Tinnitus Consultants, LLC does not contract with any third-party payors (insurance companies, HMOs, PPOs, Medicare, etc.)
Payment is expected in full by check or credit card (VISA, MasterCard, or Discover) at the conclusion of each visit. Patients are then given a document that lists and explains in detail each service rendered along with providing the appropriate diagnosis and treatment codes to submit to their carriers so they can obtain maximal reimbursement in accordance with their contracts with those carriers. Atlanta Tinnitus Consultants, LLC does not file insurance or file Medicare.
When follow-up visits for patients living at a distance from Atlanta are conducted via telephone or video conferencing, patients' credit cards are billed and the receipts (with codes) sent to them immediately.
